Our Online Application Process for Residential Care

In order to gain admission for residential care your child must meet all of the eligibility requirements for placement. If your application is accepted, you will be contacted by one of our Intake Coordinators.  A copy of your application will also be sent to your local Sheriff for his or her endorsement.

If you have any questions during the application process, please call an Intake Coordinator at 1-800-765-2697.

Additional records from your child’s school, juvenile and child welfare contacts, psychiatric or drug rehab hospitalizations, shelters and other social service sources will also be collected. Parents or guardians should also be aware that we will need to collect evidence of the child’s custody and citizenship, along with other vital records.

You should have the following documents on hand:

  • A copy of your child’s birth certificate
  • A copy of your child’s Social Security card
  • A copy of your health insurance card
  • A copy of any divorce decree, if appropriate, indicating custody arrangements
  • A copy of any psychological reports or mental health records in your possession that pertain to your child
  • A copy of any Department of Juvenile Justice (DJJ) or Department of Children and Families (DCF) court records involving your youth
  • A copy of any Individualized Educational Plan (IEP) provided to you by the school
  • A recent picture of your youth
  • A copy of your most recent Form 1040 U.S. Individual Income Tax Return for tuition calculation

To speed up the decision making process, this information can be uploaded with your child’s application; although it can also be mailed or faxed. Contact an intake coordinator at 1-800-765-2697 for the correct mailing address or fax number for your application.

The Decision Making Process

No decision can be made without our review of the records listed above, the endorsement of your local sheriff, and the information collected by your intake coordinator.  Once this information is collected, we will inform you of our preliminary decision to proceed or refer you to more appropriate community resources.

If the decision is made to proceed, we will contact you to schedule a campus visit. This is an important step because it will allow you and your child an opportunity to see the campus and meet our staff. This visit will also include an interview with both you and your child to assess your child’s commitment and your ability to participate in the therapeutic activities you will be assigned.  During this visit we will also discuss the tuition required.

Our final placement decision will be made immediately following the visit. If your child is accepted for residential placement, we will explain the remaining steps of the admission process.
